TRANSITION ASSESSMENTS

Introduction

Georgia requires that the first Transition Plan be completed before the student enters the ninth grade. Transition assessments to inform this process should begin in middle school and continue throughout the students’ high school career. It should be “an ongoing process of collecting data on the individual’s needs, preferences, and interests as they relate to the demands of current & future working, educational living and personal and social environments.” (Sitlington, Newbert & Lecoute, 1997)

Each transition planning team should include the student, parent, or any outside agency that can help the student reach the student’s measurable outcome goals along with the rest of the Individualized Education Program (IEP) planning team. These goals should be based on an individualized transition assessment process. This section of the website is dedicated to providing teachers, vocational rehabilitation counselor, administrators, and other service providers with the information, resources and planning materials needed to complete an effective “road map” to help the student successfully reach his/her measurable outcome goals after the completion of the student’s secondary career.
